  1. Print and complete the required forms listed here
  2. Please include the following additonal documents:
    1. Copy of child's certified birth certificate
    2. Copy of child's most recent physical report signed and dated by the physician
    3. Copy of enrolling parents driver's license
  3. Please include a registration fee of $60.00 per child.  Cash or check only
  4. Please include a security deposit in the amount of your child's schedule.  Cash or check only
  5. Return all forms, additional documents, and registration fees to the Center for processing.
